(RADIATOR) Note on documentation entry 6.6.3

While i was browsing the manual i noticed that in 6.6.3 ClientAttrDef
chapter the reference to the fields number is same as the "perl array
index".
That may cause some confusion since most people doesn't know that 1st
field is 0 (well, I had to check the source code).
