(RADIATOR) Duplicated Accounting Records in SQL

Hi, 

We are running Radiator with an MS SQL database to store
accounting records.   Accounting requests are sent from 
NAS devices and also from other Radiator servers (proxy)

On about 50% of the accounting requests we are seeing
multiple accounting records in the SQL table.   The duplicated
records are exactly the same apart from the ACCTDELAYTIME
field which increments by 5 seconds.

I can only assume that the NAS and accounting proxy servers 
are not receiving or being sent an accounting response and therefore
send the accounting info multiple times?

Does anyone know why this could be ?
