[RADIATOR] Support for Duo Security Two-Factor Authentication

Hello,

we are pleased to announce that Radiator now supports authentication
with Duo Security Auth API and 2-factor authentication

In typical use, the user will attempt to log in with their normal static
password.  While waiting for the authentication, their Duo Push app on
their smartphone will alert them and give them the choice of 'Approve'
or 'Deny'. Finally the RADIUS authentication will complete according to
which option the user selected.

Radiator currently supports authenticating passwords in the form
staticpassword[,factor] where staticpassword is authenticated locally
with SQL, LDAP, FILE etc. and the possible portion after the comma,
factor, is handled by AuthBy DUO. See goodies/duo.cfg for more details
and configuration example.

Support for AuthBy DUO, along with sample configuration and testing
guidelines are available in the latest Radiator patch set for Radiator 4.11.
