I should've looked more closely.  I believe it's coming from 
Authen::TacacsPlus, not Radiator.

strings /usr/lib/perl5/site_perl/5.8.5/i386-linux-thread-multi/auto/Authen/TacacsPlus/TacacsPlus.so|
grep -i authentifi

Authentification failed
