(note the Service-Type = Administrative-User) via telnet, and the NAS is 
configured to authenticate the telnet connection through radius.

The best way to deal with this is to set up a special Handler to catch these 
logins, something like this:

# define Handler to catch telnet logins

<Handler Service-Type = Administrative-User>
	.....
</Handler>

The actual cause of the problem would appear to be something in the DBD 
module that is causing a crash when given the following query:

Which appears to me to be an Accounting Start from a Cisco when it is 
rebooted, however I would need to see the preceding trace 4 debug output to 
know for sure.
