There was a recent patch that may be relevant:

2004-07-26
AcceptIfMissing did not operate as expected if a user was found but the
authentication failed..

Is it possible that the change affects you? Can you please post more details 
about of your user check items, your Radiator config file (no secrets)  and 
your Radiator log file at trace level 4?
