Hello Kevin -

I think the problem here is that you are rewriting the username.

MS-CHAPv2 uses the full username as entered by the user, including the 
realm suffix.

NB: have you included a copy of your configuration file (no secrets),
together with a trace 4 debug showing what is happening?
