(RADIATOR) CVX 1800 attributes

This question is off topic for radiator but I was wondering if anyone has 
an example of the format of access-lists attributes for the CVX.  That is, 
I want to block port 445 TCP, 80 TCP, 138 UDP to the dialed up customer on 
the CVX.  What attributes and formatting must I have to send to the CVX 
during user authentication to have this applied to their connection?  I 
can't find squat in the CVX documentation for this.  Thanks.
