(RADIATOR) Acct-Terminate-Cause, port-Error

A couple of years ago we spent a good week testing this subject. You may 
find these results useful. Our conclusion at the time was that in general it 
is very hard to try to determine what happened to a certain session from the 
Radius accounting alone. Some questions are better left unasked :)
