(RADIATOR) Radiator Crashing "Fail to add null value in not null attribute username"

Hi All

Today Radiator has started to crash every now and then. I get the following 
error

DBD::Pg::db do failed: ERROR:  ExecAppend: Fail to add null value in not 
null attribute username
DBD::Pg::db do failed: ERROR:  ExecAppend: Fail to add null value in not

Can't call method "quote" on an undefined value at 
/usr/local/lib/perl5/site_perl/5.005/Radius/AuthSQL.pm line 363.
